Dear mentor and mentee,
This page contains a Mentorship Agreement to be reviewed and discussed at your next meeting/interaction. We ask the mentees to submit the form on behalf of your pairing, based upon your mutually-agreed upon responses. Once submitted, both participants will receive an automated copy of your responses to Section 3, by email.
The intention of this agreement exercise is to allow you to establish an explicit set of expectations together, which will align and guide your partnership throughout the duration of the program. If, as your partnership evolves, you would like to revise your mentorship agreement in a substantive way, you can return to this page and create a new submission.

By submitting the form below both Mentee and Mentor agree to indemnify and save harmless Innis College, its faculty, staff and students as well as volunteers from all claims, suits and actions of every name, kind and description brought for, or on an account of, any loss, injury or damage caused by you or within your control, and release Innis College, its faculty, staff and students as well as volunteers from all personal injuries or property losses you might suffer during your participation in any activity within the Innis College Mentorship Program.
